When you fill out an intake form, we collect what you choose to share, which may include:
- Contact details — your name, email, phone, and general location (state and optional ZIP).
- For families — the kind of support you're seeking, urgency, and details about your child that you choose to provide, which can include age, diagnosis, or disability area. These fields are optional and you decide what to share.
- For advocates — your areas of focus, experience, credentials, capacity, and how you offer support.
- Anything you write in a free-text field, and whether you opt in to hear about SupportNow programs.
How we use it
- To match families with advocates who fit what they're navigating.
- To contact you about your match or your application.
- To send you information about SupportNow tools and programs only if you opt in.
What we share
We share the details needed to make a match with the advocate or family we connect you to, and you decide what's included. We do not sell your information. We may use trusted service providers (for example, to host the site or handle form submissions) who are bound to protect it.
How we protect it
We use reasonable safeguards to protect the information you share. No online service can promise perfect security, so please share only what you're comfortable with.
Your choices
- Every optional field is yours to skip.
- You can ask us to access, correct, or delete your information at any time.
- You can opt out of program emails whenever you like.
Children's information
Our forms are completed by parents, caregivers, and adult advocates — not by children. We ask for only what helps us make a good match, and details about a child are always optional.
